Applying to the Program
If you are a returner within the SLR AA program:- Fill out the Returner Application.
- Update your Human Subjects Research-certification here. When registering for the Human Subjects Research course, select St. Luke's-Roosevelt Hospital Center as your participating institution (question 1) and pick No under CME/CEU credits (question 6). On the next screen, enter your college e-mail address for institutional e-mail address, Emergency for department, and Research Assistant for role in research. Afterward, choose the basic, IRB Committee course. Attach the updated CITI certification to the online application.
- Also, if it has been over 12 months since your last PPD test, complete the Annual Volunteer Health Assessment and go to St. Luke's-Roosevelt Employee Health Services (EHS) to get a PPD test and have the form approved. You can only go on Mondays or Wednesdays because the PPD test must be read two days later (Wednesdays or Fridays).

-At St. Luke's, EHS is on the 1st floor of the Clark Building (South East corner of 114th and Amsterdam). The Nurse is available to volunteers Monday through Friday 8AM - 4PM and Thursday 8AM - 12noon. The office is closed daily from 12:30PM to 1:30PM.
-At Roosevelt, EHS is on the 1st floor, Room 1M. The Nurse is available to volunteers Monday and Wednesday from 1:00 PM - 3:45 PM, and Friday from 9:00 AM - 11:45 AM. - Please fill out the shift selection form.
- Please fill out the EmSTAT Confidentiality Form and attach it with your online application or drop it off along with any remaining paperwork at the reception desk of the Center for Student Advising, 403 Lerner Hall.
If you are new to the AA program, the following steps will guide you through the application process:
- Acquaint yourself with the AA program requirements - make sure you are interested. It is a commitment.
- The application for the program has been updated. Please fill out the New Academic Associate Application.
- After reading the Volunteer Handbook, please take the Volunteer Handbook Exam online.
- Obtain a form from your class dean (CC and SEAS) or premed advisor (GS and Barnard) confirming that you are in good academic standing. Please scan a copy of this form to attach to your online application.
- Complete the top portion of the Health Assessment Form and go to St. Luke's-Roosevelt Employee Health Services (EHS) to have it completed. Attach records of immunizations for Measles, Mumps and Rubella, and proof of a Tuberculin Test (PPD) planted within the last 12 months. Scan a copy of this document to attach to the online application.
Note: If you have not had a PPD test in over a year, you will be required to get one. You can go to the Employee Health Office at St. Luke's (enter on south side of 114th and ask the guard for directions). You can only get it done on Mondays and Wednesdays because it must be read two days later (on Wednesday or Friday). Alternatively, you can have your private doctor do it.
- Get your Human Subjects Research certification here. When registering, select St. Luke's-Roosevelt Hospital Center as your participating institution (question 1) and pick No under CME/CEU credits (question 6). Afterward, choose the basic, IRB Committee course. Save your certificate as a PDF and submit it along with the application packet.
- Fill out the EmSTAT Confidentiality Form and the HIPAA Document. Scan a copy of both of these documents to attach to the online application.
- Scan a copy of your social security card to attach to the online application.
- Scan a copy of your driver's license or Columbia ID and attach it to the online application.
- Please fill out the shift selection form and drop off your remaining paperwork at the reception desk of the Center for Student Advising, 403 Lerner Hall.
Before you submit your application, please be sure you have included ALL necessary documentation.
Note: We have moved to a full online application. Please attach the necessary documents as PDF files only. You can find scanners at the following locations at Columbia University: 305 Butler, 401 Butler, Lehman Library, Engineering Library, Avery Library, Burke Library at UTS, and the Integrated Science Library in the Northwest Corner Building. Alternatively, you may submit your documents to Columbia's Center for Student Advising in 403 Lerner with a cover page stating your name, semester applying, and new/returner status.
