Application for Degree or Certificate

Degrees are conferred three times a year:  February, May, and October.  See Academic Calendar or Thesis Deadlines for exact dates. 

Application for degree is required.

• Special note for Mac users: Please do not use "preview" to fill forms. Instead, open, fill, and save the form in Adobe.
• Here is the form: Application for Degree
• E-mail the filled form as pdf to [email protected].
• Alternatively, print, sign and return to the program office at 744A Mudd in person, by mail, fax or e-mailing a scanned form.
• Do not send the form to the department office in Fairchild or directly to the registrar. Doing so could delay the process of degree certification.

Deadlines:

August 1 for October degree
November 1 for February degree
December 1 for May degree.
