Application for Degree Certificate
Degrees are conferred three times a year: February, May, and October. See Academic Calendar or Thesis Deadlines for exact dates.
Application for degree certificate is required.
Important:
This form cannot be submitted electronically.
Please type in the required information, print, sign and return
to the program office at 744A Mudd. Do not
send the form to the department office in Fairchild or to the registrar or the
process of degree certification will be delayed.
Deadline:
August 1 for October degree,
November 1 for
February degree
December 1 for May degree.
