For corrections on certificates:

CSPA has made every attempt to provide correct certificates. If you have an error on one of your certificates, CSPA will provide a replacement at no cost. Please download the PDF correction form and fax or mail it to CSPA office.

For duplicate certificate orders

Please download the PDF duplication form and fax or mail it to CSPA office.

The first duplicate certificate is $7.95 (including postage and handling). Additional duplicates are $2 per certificate, ONLY if all duplicates are included on a single order shipped to a single address.

You may receive the discount if you order duplicates from different categories, as long as all winners are from the same publication and all requests must be made on one order.

Please read the directions carefully. All certificate orders not meeting the above criteria will be billed at the full price. Please contact CSPA with any questions.
