STUDENTS ON APPOINTMENT

Please read the following carefully and then download the appropriate forms below.


1. Nomination information form for demographic information

Complete this form using your address as of September 1. If you change your address (or any other demographic data) at any time, be sure to make the change in your on-line listing www.my.columbia.edu. It is also helpful to inform the departmental office of the change.

2. W4

The address listed on your W4 is the address to which your W2 will be sent in the spring. It also appears on your paycheck. IT IS YOUR RESPONSIBILITY TO MAKE SURE THAT IT IS CORRECT. To update your address, tax information and view your paycheck, go to www.my.columbia.edu. We are not permitted to give tax advice so please take care when completing your forms.

3. IT2104 New York State tax form

If you are not a New York State resident indicate this on your nomination form and ask Pam or Valencia for an IT2104E.
 

FOR US CITIZENS AND PERMANENT RESIDENTS ONLY
 

4. I-9 Employment Eligibility Verification Form

You will need to complete Section 1 of the I-9 Form at www.newI9.com.

You must bring the appropriate original documents with you to complete Section 2 of the I-9 in person at the Service Center located in 210 Kent Hall.

The I-9 FORM MUST BE COMPLETED AND ON FILE BEFORE YOU CAN BE ENTERED INTO THE PAYROLL SYSTEM.

NOTE: If you are a returning student and were not on payroll in 2008-2009, you must complete a new l-9 form. Please follow the instructions listed above.

5. GSAS only: Work-study application for 2009-2010

YOU MUST COMPLETE THIS FORM even if you are not planning to apply for a loan or Work study position. Return the form to 602 Philosophy for signature. Students outside of English and Comparative Literature should have the form signed in their own departments.

6. GSAS only: Free Application for Federal Student Aid (FASA)

Apply online at www.fafsa.ed.gov. GSAS code: E00113.

Your fellowship letter from GSAS will include information on the application to the NYS Tuition Assistance Plan.
 

INTERNATIONAL STUDENTS
 

7. I-9 Employment Eligibility Form

International students must go to the I-9 Service Center located in 210 Kent Hall to complete the I-9 Form. Please remember to bring the appropriate original documents with you when you visit the Service Center. It is best if you can do this before you leave for the summer, but if this is not possible, you must do it as soon as you return in the fall and in no case later than 72 hours after the first day of classes. You cannot be paid for any days worked before this 72-hour deadline.

8. Your home country may have a tax treaty with the US. Check the ISSO website for full information. Tax treaties can also be applied to stipends, which are otherwise taxed at 14%. Once you have the section number of the law that applies to your country you can file it with Payroll and with Student Financial Services.
 

DISBURSEMENT
 

1. STIPEND

Two-thirds of your award comes as a stipend which is not reported on your W2 but which should be reported on your tax return. The stipend will be disbursed in two payments: early September and the middle of January.

2. SALARY

The remaining third of your award comes as a semi-monthly salary check on the 15th and last working day of each month, September through May. Checks may be directly deposited in your bank or picked up from 602 Philosophy. If you would like direct deposit, go to http://managers.hr.columbia.edu/tig/direct-deposit.  We request that you pick up your monthly salary check promptly.  If you neglect to pick up your check in a timely manner, it will be your responsibility to contact the payroll department with regard to re-issuing an outdated salary check.
