What to Bring to the Deposit  



When all revisions are completed, deposit of the final thesis copies may be made at the Dissertation Office in 107 Low Library, Monday – Friday 9:00am-4:45pm. The following items must be brought to the deposit. Normally, the student receives the necessary deposit forms at the time of the defense. If needed, most of the forms are available online. If you deposit in the academic year following your defense please ensure that you have up-to-date deposit forms by downloading them at the above link.

  • TWO UNBOUND deposit copies of the dissertation, in separate boxes, which conform to all the layout requirements specified in these guidelines and which are printed single-sided on at least 25% cotton and 20 lb weight white paper (five copies if the commercial publication is chosen.)

  • ONE extra copy of the abstract and title page. The author should type the name of the sponsor on the extra copy of the title page beneath his or her name. Please type the word “Professor,” “Sponsor” or “Advisor” before typing the name. Do not use staples on these documents. These extra pages are those referred to in the ProQuest “Publishing your Dissertation” booklet. The extra copes may be on regular printing paper.
  • FEDERAL SURVEY OF EARNED DOCTORATES.

  • CAREER SERVICES QUESTIONNAIRE (optional)

  • 4 ½” x 7” APPROVAL CARD signed by the sponsor and the Department/Program Chair. If the card is unsigned at the time of deposit, please send all other materials to the Dissertation Office while efforts are being made to have the card signed. The deposit is not complete until the card is completed and submitted to the Dissertation Office.

  • ProQuest Public Availability of the Dissertation form authorizing ProQuest to publish the dissertation and/or act as agent for copyright registration. Pages 3 and 4 in the ProQuest “Publishing your Dissertation” booklet are required, pages 5-6 are optional.

  • EMBARGO FORM (optional.) The Embargo form is required only in the unusual circumstance that the author wishes to withhold the dissertation from publication for a specific period of time.

  • MONEY ORDERS or CERTIFIED CHECKS to cover the necessary fees.
  • The deposit fee of $160 for traditional publishing or $255 for open access publishing, payable to Columbia University, must be in the form of a separate money order or bank certified check. Personal checks, credit cards, cash and international postal money orders are NOT accepted.
  • The optional $65 fee payable to ProQuest to register the copyright may be combined with other payments, such as for the discount author copies of the dissertation, to ProQuest into one check. Unlike the Dissertation Office, ProQuest does accept personal checks.

    DO NOT COMBINE THE PUBLISHING FEE ($160 or $255) WITH FEES TO ProQuest.
