Obtaining a Card


Columbia University Officer and Staff

Please note: the ID Center cannot issue a Columbia Card ID to new employees without a Social Security Number (SSN). Employees who have applied for an SSN but have not received it may be issued a special ID Card until it has been obtained.

Morningside Campus

Your departmental administrator or departmental human resources representative will need to make the request through the WAFFIL database. This request will create a UNI and an email account, as well as authorize the creation of a Columbia Card. (Please note: CUIT will not produce a second UNI if your DA is updating your records.) If you are unsure who in your department has access to WAFFIL, email us [AskUs] or call 212-854-5883 and we will be happy to assist you.

Once this has been done, you will need to come to the ID Center in 204 Kent Hall to have your card created and issued.

Columbia University Medical Center

Please note: there are two forms of ID in use at the Columbia University Medical Center (CUMC): the Columbia Card and the CPMC ID.

A CPMC ID is required for all hospital personnel, whether they are students at Columbia University or not. In order to enter the Columbia Presbyterian Medical Center, this ID must be shown.

The Columbia Card is the official University ID for students, faculty, and staff on all campuses, but it alone does not allow entry into the CPMC under any circumstances. Alternately, possession of a CPMC ID does not guarantee access into any buildings on either campus that are exclusively University property (such as the Dodge Fitness Center). Students and staff at the Medical Center must have both IDs in order to access both the CPMC and University buildings.

The Security Department at the Medical Center issues, replaces, and validates CPMC badges for staff and students. For more information on CPMC ID, please contact Health Sciences Security at (212) 305-8100 or visit them in the Black Building, Room 109, 650 West 168th Street.

Lamont-Doherty Earth Observatory

You must contact either Jennifer Luken or Joanne Domenick at Human Resources in room 205 Administration. If you have any questions, please contact them at (845) 365-8457. They will process your request.

Once this has been done, you will need to come to the ID Center in 204 Kent Hall on the Morningside campus with valid picture ID (state-issued license or passport only) in order to have your card created and issued. Each officer card is valid for three years. Please refer to Replacement Card for information on how to renew an expired officer card, or how to replace a lost/stolen officer card.

St. Luke's Hospital

Your appointment must be processed through the Office of Graduate Medical Education (GME) in room 2069 of the Plant Building. The GME office compiles a list to forward to the Office of Faculty Affairs on the Health Sciences campus for authorization. Any changes or corrections to the list must be done through the GME office. If you have any questions, please contact the GME office at (212) 523-2154.

Once you have been authorized by the GME Office, you will need to come to the ID Center in 204 Kent Hall with valid picture ID (only passports or state-issued licenses will be accepted as valid) to have your card created and issued. Each officer card is valid for three years. Please refer to Replacement Card for information on how to renew an expired officer card, or how to replace a lost/stolen officer card.

Harlem Hospital

You must obtain an ID authorization form from the Affiliation HR office in room 6208 of the MLK Pavilion. If you have any questions, please contact them at (212) 939-1720 or 939-1723.

Once this has been done, you will need to come to the ID Center in 204 Kent Hall on the Morningside campus with the signed authorization form and valid picture ID (only passports or state-issued licenses will be accepted) to have your card created and issued.
