Replacement Card

Morningside Campus

To renew an expired card: Come to the ID Center in 204 Kent Hall with your expired card and we'll issue a renewal card. Any swipe access you had on your expired card will still be in effect on your renewal card.

To replace a lost/stolen card: Your departmental administrator or departmental human resources representative will need to validate your employment status in the WAFFIL database. If you are unsure who in your department has access to WAFFIL, call 212-854-5883 and we will be happy to assist you. 

Once this has been done, you will need to come to the ID Center in 204 Kent Hall to have your card replaced. Any swipe access you had on your lost card will be deactivated and transferred to your replacement card. In some cases, this may take up to 24 hours to process.

Medical Center

To renew an expired card: Come to Student Administrative Services at 1-141 Black Building with your expired card and we'll issue a renewal card. To renew an expired CPMC badge, please visit the Security Office in room 109 Black Building. 

To replace a lost/stolen card: Your departmental administrator or departmental human resources representative will need to issue you a letter requesting the replacement of a lost/stolen Columbia Card. If you are unsure who is authorized to sign this letter, contact Student Administrative Services at 212-342-4790.

Once this has been done, you will need to come to Student Administrative Services at 1-141 Black Building with the signed authorization letter to have your card replaced. To replace a lost/stolen Columbia Presbyterian Medical Center badge, please visit the Security Office in room 109 Black Building.

Lamont-Doherty Earth Observatory

To renew an expired card: Bring your expired card to Human Resources in room 205 Administration Building. They will forward the card to the ID Center for renewal. You may then come to the ID Center in 204 Kent Hall on the Morningside Campus with your expired card and we'll issue a renewal card.

To replace a lost/stolen card: Go to Human Resources in room 205 Administration Building. They will process the paperwork for your card replacement so that you may then come to the ID Center in 204 Kent Hall on the Morningside Campus, and we'll issue a replacement card.

St. Luke's Hospital

To renew an expired card or to replace a lost/stolen card: Your appointment must still be in effect at the Office of Graduate Medical Education (GME) in room 2069 of the Plant Building. The GME office compiles a list to forward to the Office of Faculty Affairs on the Health Sciences campus for authorization. Any changes or corrections to the list must be done through the GME office. If you have any questions about your status, please contact the GME office at (212) 523-2154. If your appointment is still active, come to the ID Center in 204 Kent Hall on the Morningside Campus, and we'll issue a replacement card.

Harlem Hospital

To renew an expired card or to replace a lost/stolen card: You must obtain an ID authorization form from the Affiliation HR office in room 6208 of the MLK Pavilion. If you have any questions, please contact them at (212) 939-1720.

Once this has been done, you will need to come to the ID Center in 204 Kent Hall on the Morningside Campus with the signed authorization form to have your card replaced.
