All groups applying for membership in the IGB must submit a membership application to the IGB containing the following information:
1. Cover page including all relevant background information and statement of purpose which demonstrates fulfillment of the criteria for membership contained herein
2. IGB Questionnaire
3. Constitution and by-laws including the outlined articles
4. Budget including potential sources and uses of funds
