Procedures for Getting Eye Exams

Staff represented by a union contract and who regularly work at computers are eligible for an annual eye exam at St. Luke's Hospital. This includes: 1199 members, 2110 members and non-union support staff. The program is not available to staff members with officer status.

The eye exam program is administered by the Benefits Office in 1901 Interchurch Center (870-3074). That office has copies of a memo describing the program (including instructions for scheduling appointments) as well as the necessary forms.
