2. How do I view my paycheck?? |
To view (and print) your paycheck please visit the myColumbia website which is designed to enable Columbia's faculty, staff, and students to find the information and resources they need quickly and easily in one central place. To access the myColumbia portal please follow the instructions below:
- Go to www.columbia.edu
- Click on "Faculty & Staff" link on the left
- Click on "myColumbia" link on the right
- Log in with your UNI and password

3. Who do I contact if I don't receive a W-2 tax form? |
Call the W-2 Hotline at 212-854-4624. You must email (w2@columbia.edu) or fax (212-854-1027) your request and include the following information: your full name, SSN, current address and telephone number.

4. Who do I need to inform if I change my address and telephone number? |
In order for your mailing address to be changed officially in the system you must complete a Change of Address Form. Contact Ms. Jackie Martinez (612 SSW, x12286, jmm125@columbia.edu) with your new address and phone number and request to have a form mailed to you. You may also pick up a form from Ms. Bienvenida Alvarez (606 SSW, x12355, ba4@columbia.edu).

5. How do I get an ID card? |
Contact Ms. Bienvenida Alvarez (606 SSW, x12355, ba4@columbia.edu) to make the request through Waffil. This one request will create a UNI and an email account, as well as authorize the creation of a Columbia Card. Once this has been done, you will need to go to the ID Center in 204 Kent Hall to have your card created and issued. Each officer card is valid for two years.

| 6. How do I replace my ID card? |
To renew an expired card you will need to go to the ID Center in 204 Kent Hall with your expired card and they will issue a renewal card. Any swipe access you had on your expired card will still be in effect on your renewal card.
To replace a lost/stolen card contact Ms. Bienvenida Alvarez (606 SSW, x12355, ba4@columbia.edu) to validate your employment status in Waffil. This one request will create a UNI and an email account, as well as authorize the creation of a replacement Columbia Card. Once this has been done, you will need to go to the ID Center in 204 Kent Hall to have your card replaced. Any swipe access you had on your lost card will be deactivated and transferred to your replacement card. In some cases, this may take up to 24 hours to process.

7. How do I set up my email account? |
- Go to www.columbia.edu
- Click on "E-mail & Computing" on the left-hand side of the screen.
- Click on "My UNI" under quick links.
- Click on "Activate a New UNI Account".
- Read the Computer and Network Use Policy and click "Accept" on the bottom of the screen.
- Read Your Responsibility on the Columbia Network and click "Accept" on the bottom of the screen.
- On the page entitled Activate Your Account read and fill in the requested information for selecting a password.
- Click on "Create My Account" on the bottom of the screen when you are finished filling out the requested information.
- Your e-mail is UNI@columbia.edu.

| 14. How do I get copies made? |
Full-time Faculty: Copiers are located in rooms 721, 821, 925 and the 11th floor hall. All copiers require an access code to make copies. Please contact Mario Pereira (103 SSW, x12212, mgp8@columbia.edu) to receive your access code.
Adjunct Faculty: A copier is located in the Adjunct Faculty Office (309 SSW).
Full-time and Adjunct faculty may also leave requests for copying with the faculty assistants or with Mr. Mario Pereira in the Copy Center (103 SSW). Please fill out a copy request form located on the counter with each copy job you submit.

| 17. How do I order desk copies? |
Desk Copies can be ordered by faxing your request to the publisher on CUSSW letterhead. Also, some orders can me placed on-line via the publisher's website. However, please note that many publishers are not willing to provide desk copies until a course book order has been placed at the bookstore. Instead, they issue exam or review copies, which requires payment if an order is not placed at the bookstore or if it is not returned to the publisher. Please contact the faculty assistants in 725 SSW and 308 SSW for any needed assistance on ordering desk copies.

Contact Campus Course Paks, Inc.: (1-800-254-PAKS, www.ccpaks.com). Processing takes 28-35 days to clear copyright restrictions.

Grades should be submitted online. Please follow the instructions below to enter the Web Grading site:
- Go to www.columbia.edu
- Click on the "Faculty & Staff" button
- Click on "Student Services Online"
- Log on with your University Network ID (UNI)
- Choose "Web Grading"
You may also access Web Grading at the CourseWorks site by logging in your UNI and password.

| 20. If a class falls on a religious holiday, does the class still meet? |
YES. It is the policy of the University to respect its members' religious beliefs. In compliance with New York State law, each student who is absent from school because of his/her religious beliefs will be given an equivalent opportunity to register for classes or make up any examination, study, or work requirements that he/she may have missed because of such absence on any particular day or days. No student will be penalized for absence due to religious beliefs and alternative means will be sought for satisfying the academic requirements involved.
Officers of administration and instruction responsible for scheduling of academic activities or essential services are expected to avoid conflict with religious holidays as much as possible. If a suitable arrangement cannot be worked out between the students and the instructor involved, they should consult the appropriate dean or director. Any additional appeal may be taken to the Provost.

| 21. If I have a student with a disability in my class, are there accommodations I should make? |
NO, accommodations are only to be made when the instructor has received written information from the appropriate office. In order for the enforcement of the ADA, all accommodations are made by the University Disability Officer. Faculty members are not permitted to make individual disability accommodations. Students who require accommodations in class and/or field instruction must identify themselves by completing a Columbia University Disability Identification Form. This form, included with materials sent to each student upon acceptance to CUSSW, is also available in the CUSSW Coordinator's office (526 SSW) and the University Disability Services Office (Lerner Hall, 8th Floor).
In addition to completing the Disability Identification Form, students requesting accommodation must provide current and appropriate medical and/or diagnostic documentation as well as a medical release form. In cases involving students with learning disabilities, appropriate documentation must provide a current assessment of the student's learning style and skills, and must include a report summary and complete test battery scores.
The University Disability Services Office must receive all of these materials before any accommodation request can be considered. Once a decision has been made, Ms. Karma Lowe (526 SSW, x12364, knl3@columbia.edu) will inform instructors of the appropriate accommodations.

| 34. How do I send an email to all my students? |
You may send emails to your students by logging on to CourseWorks with your UNI and password. Once you have logged in, click on "Class Email". You then have the option of sending emails to individual students, a group of students, or the entire class.

| 35. How do I place course materials on the web? |
Please submit all syllabi to swcourses@columbia.edu. The Office of Computing and Information Technology will place all syllabi online through Courseworks for you and your students to access. To access syllabi go to https://courseworks.columbia.edu/ and log in with your UNI and password.

| 36. How do I place readings on reserve? |
When you submit your syllabus to swcourses@columbia.edu the Social Work Library will review your syllabus and place required readings on reserve. Reserve materials for all sections of a given course will be listed under the Library Reserves tab in CourseWorks, including links to any books or articles available in electronic format.
If an article is unavailable in electronic format, they will let you know via email and ask you to submit a photocopy so they can convert it to electronic format.

| 38. Is there a policy regarding teaching absences? |
It is the responsibility of the instructor to meet all scheduled classes. In the event of an unavoidable absence due to emergencies, religious holidays, or other scheduling conflicts, the instructor must reschedule missed classes, arrange for a replacement, or provide alternative instruction. If an instructor must cancel a class due to an emergency or illness, he/she must contact Jackie Martinez, Administrative Coordinator of the Faculty Support Center, Ms. Lisa Cammett, Faculty Assistant, as well as the Receptionist, Ms. Noris Schrils. Faculty should also send an email message to all students in their classes. Should an instructor be absent for an extended period due to illness or injury, the instructor should notify the Associate Dean's office so that a substitute instructor can be provided.

| 39. If a student in my class is having difficulty with his/her writing skills, what resources are available? |
The CUSSW Writing Center is a service for matriculated students who desire assistance with their course writing assignments. Students may use the service for help with any writing issue-from conceptual problems, such as developing, clarifying, and organizing thoughts-to questions about grammar, punctuation, usage, APA style, and integrating literature into their papers. They may also use the service to discuss how to move forward with a particular paper, how to narrow or expand their focus, and how to organize and structure material.
The Center also conducts workshops on APA style and writing strategies, and it provides handouts on a variety of writing issues, both on its web site (www.columbia.edu/cu/ssw/write) and in racks located directly outside the Center (530A SSW). The Center's web site also provides links to various research and organization sites to aid students in their research and writing requirements.
