Elizabeth Alvarado, LCSW-R, is currently the Social Work Director for Isabella Visiting Care, Inc. for home and community based services.  She has more than 20+ years of clinical experience working for New York Presbyterian Hospital and the Visiting Nurse Service of New York.

Her professional work experience in the field of social work includes discharge planning across the continuum of care, end of life care, group work with oncology clients, case management, culturally competent social work practice, and working with people who are living with traumatic brain injuries.  She oversees five distinct community based programs serving the frail homebound elderly, caregivers, people with mental health disorders and the chronically ill with physical and cognitive disabilities.

She was published in the CMSA Case Management Book, 2007.  She received her bachelor’s degree in community health education from Hunter College, and went on to pursue her social work graduate degree from Fordham University in 1997.  She has completed two postgraduate certificate programs: one from NYU for bioethics and medical humanities, and another from SBW Partners in gerontology.
