We have revised the field evaluation forms to be used at the end of the spring 2012 semester. Please use the following versions.

Note that your field instructor is now required to e-mail a copy of the completed Word document to swfeval@columbia.edu. This is in addition to depositing the signed printed form, plus one photocopy, with the Field Education Department. See the instructions on the last page of the form.
