All students must complete and submit this form in order
to initiate the financial aid process. When completing
this form, students should read each question carefully
and be sure to indicate the type(s) of aid for which
they are applying.

CUAPP for PhD
After all sections have been completed, students should submit
the CUAPP to the financial aid office as early as possible
to avoid delays in processing. All other required documentation
may be sent in thereafter.
