Overview
Submit your completed MMR form to the Immunization Office in Wien Hall by August 31, 2009. Please note: this form requires a physician's signature.
New York State public health law requires that students taking six or more points provide documentation of immunity to measles, mumps and rubella (MMR) before their first term of study. There are several ways to document immunity. In all cases, the Columbia University MMR form must be completed and submitted to the Immunization Office in Wien Hall no later than August 31, 2009.
Completed forms may be submitted in the following ways:
By mail: Immunization Office, Wien Hall, 411 W. 116th Street, MC 3712, New York, NY 10027
By fax: 212.854.5078
If you fax your documentation, please make sure to include your full name, date of birth, CUID, and school you will attend on the form. It cannot be processed without this information. We cannot verify receipt of individual documents so please keep a copy of your fax confirmation. We do provide email confirmation later in the semester.
In person: You may also drop off your immunization documentation. The Immunization Office is located in Wien Hall, first floor. The office is open Monday through Friday, 9:00 a.m. – 5:00 p.m.
