Overview

Students must submit completed immunization documentation to the Columbia Health Immunization Office for MMR prior to registering for classes. It is recommended that students complete this requirement 30 days prior to their scheduled registration.

New York State public health law requires that students taking six or more points provide documentation of immunity to measles, mumps, and rubella (MMR) before their first term of study.

There are several ways to document immunity. In all cases, the Columbia University MMR form should be completed and submitted to the Immunization Office before registration begins. 

Completed forms may be submitted in the following ways:

In Person: Columbia Health Immunization Compliance Office is located at 108 Wien Hall. Please check our hours of operation. 


NOTE:You will need to submit completed immunization documentation for MMR before you can register for classes. All incoming students are placed on hold prior to their first registration.
