Overview
Submit your completed MMR form to the Columbia Health Immunization Compliance Office before registering for classes (August 1, 2011).
New York State public health law requires that students taking six or more points provide documentation of immunity to measles, mumps, and rubella (MMR) before their first term of study. There are several ways to document immunity. In all cases, the Columbia University MMR form must be completed and submitted to the Columbia Health Immunization Office at Wien Hall no later than August 1, 2011.
Completed forms may be submitted in the following ways:
By mail: Immunization Office, Wien Hall, 411 W. 116th Street, MC 3712, New York, NY 10027 By fax: 212.854.5078 If you fax your documentation, please make sure to include your full name, date of birth, CUID, and the school you will be attending on the form. It cannot be processed without this information. We cannot verify receipt of individual documents so please keep a copy of your fax confirmation. NOTE: Please keep a copy of your immunization documents for your records. |
