COLUMBIA UNIVERSITY FLU NET HEALTHCARE PROGRAM
- MOBILE PROGRAM TERMS AND CONDITIONS


Columbia University’s mobile SMS program runs on the mobile short code 43186. Columbia University will NOT charge you to use this service; however, your Wireless Service Provider may charge for sending and/or receiving messages and for air-time.

SERVICE TERMS:

1. Columbia University’s FLU NET Mobile Program includes sending SMS messages, an IVR call and a SMS survey application (collectively the 'Service'). Note: To process the SMS requests for this Service, you may be charged a fee to send and receive those SMS messages based on the terms of your Wireless Service Plan. All charges are billed by and payable to your Wireless Service Provider. Check with your Wireless Service Provider if you have questions about your Service Plan. Message, Data & Airtime rates may apply.

2. Columbia University is not liable for any delays in the receipt of any SMS message, as delivery is subject to effective transmission from your Wireless Service Provider/Network Operator.

3. This Service is available on the following Wireless Service Providers: Alltel Wireless, Appalachian Wireless, AT&T, Boost USA, Cellcom, Carolina West Wireless, CellularSouth, Cincinnati Bell, Cellular One Dobson , Iowa Wireless, Midwest Wireless (Alltel Wireless), Nextel Communications, nTelos Wireless, Sprint PCS, T-Mobile USA, Unicel, U.S. Cellular, Verizon Wireless, Virgin Mobile USA.

To participate the Columbia University’s FLU NET Mobile Program please call 1-800-895-7460 and provide the pin on the back of your FLU NET card. Once you have completed the questions within IVR service you will be opted in to the SMS survey program.

To STOP (opt out of) the Columbia University FLU NET Mobile Program: Reply 'STOP FLU' to any message, or text 'STOP FLU' to 43186.

To get HELP on the Columbia University FLU NET Mobile Program: Reply 'HELP FLU' to any message, or text 'HELP FLU' to 43186.

To get HELP on services that run on the 43186 short code: Text 'HELP' to 43186.

To STOP (opt-out of) ALL services on the short code 43186: Text 'STOP', 'END', 'QUIT', 'UNSUBSCRIBE', 'CANCEL', or 'STOP ALL' to 43186.

When you complete forms online or otherwise provide Columbia University information in connection with this Service, you agree to provide accurate, complete, and true information. You agree not to use a false or misleading name or a name that you are not authorized to use. If Columbia University, in it's sole discretion, believes that any such information is untrue, inaccurate, or incomplete, you may be refused access to the Service and Columbia University may pursue any appropriate legal remedies.
