Overview


Faculty and staff with permanent or temporary medical conditions may have a spectrum of needs that can be addressed by Disability Services and the Return to Work Program:

  • To arrange permanent or temporary accommodations in your workplace.

  • To assist you in understanding the benefits available to you during a medical leave and the documentation you need to submit.

  • To ease your transition from a medical leave back to a job with the University.

Whatever your circumstances, we encourage you to contact Disability Services or the Return to Work Program for a confidential consultation.

Whether you are returning from a medical leave, have a chronic condition, or need an accommodation for a temporary injury or the effects of a medical procedure, we can coordinate with your department, when appropriate, to get you what you need. University policies conform to the provisions of the Americans with Disabilities Act.

If you need a workplace accommodation, submit an Accommodation Request and a Disability Release to the Disability Services office. (These are available in the Forms box at right.) Please contact Disability Services if you have any questions.

The Return to Work Program facilitates the return of employees of the University who are out of work due to disability (non-occupational) or workers compensation. The program provides outreach and case management to all employees as well as referrals to various programs and services as needed.

Please remember to complete all applicable paperwork (e.g., the New York State Disability Form DB450) when going on a leave. For general information about leaves, Officers can consult Benefits in Brief; union-affiliated Support Staff should refer to the appropriate collective bargaining agreement.
