The verse is commonplace. But the harsh sounds of k and d -- kyaa , zadah , dil , kaun , dard , kaa , daar -- have made the impression of sorrow and suffering even more firmly established.

For maa))il-e aazaar there are two meanings: one is 'loving affliction' and the other is 'inclined toward the bringing of affliction'. In this way the heart itself is difficulty-stricken, and is also active in bringing trouble upon the speaker.

It's a strikingly insha'iyah verse, with both lines taking the form of rhetorical questions. And the exclamatory energy comes through very clearly. The speaker isn't scolding the heart directly, but he's certainly complaining vigorously to somebody else about its incorrigible masochism and bad behavior.
