dil-e be-taab aafat hai balaa hai
jigar sab khaa gayaa ab kyaa rahaa hai

1) the agitated/faint heart is a calamity, it is a disaster
2) it ate up the whole liver-- now what has remained?!

Notes:

be-taab : 'Faint, powerless; agitated, restless, uneasy impatient (= be-chain ); devoid of splendour, lustreless'. (Platts p.202)

S. R. Faruqi:

The opening-verse is by way of introduction, but this theme is not devoid of pleasure-- that for the heart to become blood, there was need for more blood. This it obtained from the liver, and in this way the heart destroyed the liver itself.

There's also an especially complex evocation of the 'kya effect', made possible by the sheer, ambiguous simplicity of kyaa rahaa hai :

=Has it remained? (a yes-or-no question)

=What has it remained? (what shape has it settled into?)

=What has remained? (is there anything left, or has it vanished entirely?)

=As if it has remained! (it hasn't, of course!)

=What it has remained! (what an extraordinary state it's in!)
