and teaching outcomes through documentation on the Interdisciplinary
Patient and Family Education Record.
Health care providers
also discuss learning needs and educational outcomes with members
of the health care team including: physicians, nurses, dietitians,
respiratory therapists, physical therapists, occupational therapists,
social workers, home care staff, child life staff, and pharmacists,
etc.

collaboration

through referrals to other disciplines, discharge planning conferences,
or patient care conferences
