Assess for General knowledge about the illness/reason for treatment,
proposed plan of care, and health promotion.

Using open-ended questions, ask what the patient knows about the illness. Example: "Tell me what you know about…."·

Clarify misconceptions and provide additional information as needed.

Identify the patient's and family's learning needs and concerns about hospitalization, diagnosis, illness, treatment, medications, drug-food interactions, safety, procedures, equipment use, community resources, follow-up care, health maintenance and health promotion.

Foster appropriate follow-up.
