It is 5:45 PM and you’re still in clinic. Your last patient, a new visit, is a 26 year old G1P1 s/p BTL who has arrived late (in more ways than 1) c/o amenorrhea. You have tickets to the Mets (but consider w/ups like metrorrhagia foul play)
What is the next step?
Make a surreptitious exit in as few steps as possible
Tell your secretary:”Reschedule, no more patients, period-unless they’re having regular periods”
Order every endocrine lab under the sun, including rT3, and refer to Endocrine (It’s always the thyroid)
Ask your nurse to perform a test and do a focused review in Uptodate while you’re waiting for the result which will be available within 5 minutes
