Planning Sheets & Forms for Undergraduates
Major and concentration requirements worksheets are intended to help students plan their coursework and completion of their program. These are not official documents and do not need to be formally signed by the DUS. However students are strongly encouraged to make an appointment to go over their degree progress with the Undergraduate Program Coordinator. Please do not rely on the SSOL degree audit. The degree audit is often inaccurate.
These forms are all fillable pdfs. You may type directly into the text fields and save and/or print the form. If you have any issues saving your text to the form, try printing to pdf. Alternately, forms may be printed and completed by hand.
