download the following Matlab 2008a files: 
simu.m
table.m
findvh.m
primal_form.m (optional)

Then run the Matlab program table.m. This program calls simu.m and findvh.m. (The symbolic expressions for the derivatives of the Lagrangian given in findvh.m were produced with primal_form.m.)
