• PMH: HTN, Aortic Stenosis

• Meds: Lisinopril 20 mg po daily, Toprol XL 200 mg po daily, HCTZ 25 mg po daily, ASA 325 mg po daily

• ROS: Occasional CP, palpitations, light headedness

• Family History: HTN, DM
