• Meds: Glipipizide, Metformin, Ramipril, Atenolol, Furosemide, ASA

• VS: T 101.3 BP 95/50 P 140 R 26 O2 sat 96%

• HEENT: A/V nicking, dry mucous membranes

• Neck: flat JVP

• Chest: No rales

• COR: S4, no murmurs

• ABD: Soft, non-tender

• Skin: Poor skin turgor

• Neuro: Somnolent, no focal deficits
