Chief Complaint: 37 year old man presents to clinic complaining of 6 month history of polydipsia and polyuria. Patient drinks up to 16 liters/day and urinates up to 24 times/day. He denies history of headache, visual changes, medications, or alcohol. He also denies history of head trauma, DM, renal disease, or psychiatric disease.

PMH: non-cardiac CP syndrome
Meds: None
VS: T 98.6 BP 122/76 P 88 WT 80 kg
HEENT: PERL, discs sharp, flat; moist mucous membranes
Neck: flat JVP
Chest: No rales
COR: regular, no S3/4, no murmurs
ABD: Soft, non-tender
Skin: good skin turgor
Neuro: VFF to direct confrontation
