Event Approval Form (EAF)

Instructions:

Fill out all spaces, including contact info. Please give your rep at least 48 hours to respond. If you do not hear from him or her by then, contact the ABC exec board.

If you are submitting this as an appeal, please note this somewhere in the description section of your EAF.

If you are a Category A group, you must submit your EAF appeal by noon on Sundays. You must wait until the following Wednesday regarding the status of your EAF appeal, since your EAF appeal must be approved by the entire ABC board at our weekly meetings.

If you are receiving funding from sources other than ABC, we will require verification of them in writing prior to approval.
