Program Overview:
Healthy Schools Healthy Families (HSHF) is an elementary
school-based health promotion program for medically underserved children
in New York City. HSHF identifies and addresses unmet health needs
in the school community by partnering with the schools, the surrounding
community, local and national stakeholders and private and public
sectors. The program is led by NewYork-Presbyterian Hospital, Columbia
University Community Pediatrics, and Weill-Cornell Medical Center.
HSHF started in 2004 and is now in seven elementary schools located
in Harlem, Washington Heights, and Inwood. HSHF is based on the CDC
coordinated school health program model to blend the strengths of
an academic medical center, public schools, and community resources
to tailor a sustainable programmatic approach to health promotion
and elicit long-term impact.
Mission/Goals:
The mission of HSHF is to promote healthy lifestyles and
mental well-being in the school community. HSHF uses a school-based
decision making model and works with community partners to achieve
its five goals:
• Cultivate a school culture which values physical activity,
healthy eating, and mental wellness
• Achieve environment and policy change that promotes healthy
lifestyles and supports healthful behaviors
• Facilitate access to services for children with health needs,
particularly children with asthma
• Facilitate insurance enrollment for children and families
• Ensure 100% of children have completed their required immunizations
Number of people served per year:
HSHF serves approximately 4500 students and their families
and the school staff of all seven of its beneficiary schools.
Outcomes/results:
• Asthma & Flu Outreach: Last academic year, all
seven HSHF schools received flu and/or asthma education workshops
• Immunizations: Last year marked the sixth consecutive academic
year that all HSHF schools met or exceeded the Department of Education
requirements of 97% student body immunization compliance.
• Nutrition Education: Over the 2009-2010 school year, HSHF
held 352 nutrition events yielding 52,277 participant encounters.
A survey of nutrition knowledge and behavior documented improved scores
in HSHF 4th and 5th graders.
• Physical Activity: This year the seven HSHF schools are averaging
110.8 minutes/week/class of physical activity. This is a 76% increase
from last year.
